We look forward to welcoming you to Sprout Pediatrics.
Your choice of pediatrician is very personal, and collaborating to support your children's health and wellness works best with shared expectations. If you have questions and haven't already read How Sprout Works, please do so. Our immunization policy is detailed and answers most vaccine questions. Lilay is happy to answer almost any question about Sprout. We want you to be comfortable choosing Sprout as your medical home.
Please check whether we are currently accepting new patients and accept your insurance plan; note that we can’t verify that we are on your specific plan, only that we are providers or a medical home with an insurance company. Please call your plan and ask if Sprout Pediatrics &/or Dr. Kemmerle are covered and verify your benefit tier and copay information. We don’t like surprise bills any more than you do. If you want to bring your children to Sprout please contact Lilay using the form below. Be sure to tell us how you found us or who referred you. We will email you to answer questions and register your kids or add them to our waiting list.
When we’re able to register your kids we’ll email a link to create accounts on our AdvancedMD patient portal and Spruce, our secure messaging app. We’ll assign fun medical, consent & administrative forms in the portal, then request past medical records and schedule a new patient visit for all children over 4 months old. We’ve found that meeting and examining your child, having timely vital signs and measurements in their chart, and reviewing past medical history before an urgent question or problem pops up allows us to offer advice tailored to your child when something does happen. Finally, if your health insurance requires a medical home remember to change it to Sprout before your first appointment.
Please read our Immunization Policy, see whether we are accepting new patients & make sure we accept your Insurance, then complete the form below if you would like your children to be Sprout Pediatrics patients. Thank you!
